Abstract

Hypertension and left ventricular hypertrophy are majör cardiovascular risk factors that contribute to increased morbidity and premature mortality. Nondipper hypertensive patients (patients who do not show nocturnal blood pressuer fail have end organ damage more than dipper ones. QT dispersion that is caused by in homogen repolarization of the ventricle predispose individuals to ventricular arrhytmias. Aim of this study is to determine if there is any relationship between the dipping status and QT dispersion in essential hypertensive patients, MATERIAL AND METHODS: 63 patients with essential hypertension are included in this study. Patients were divided into two groups by ambulatory blood pressure monitoring for 24 hours: dippers (n:27) and nondippers (n: 36). Presence of left ventricular hypertrophy is determined with electrocardiography and echocardiography. QT dispersion (QTd) and corrected QT dispersion (QTc) are measured for both groups. RESULTS: There was no difference for the presence of left ventricular hypertrophy between dipper and nondipper hypertensives (p>0.05). QTd and QTcd are found to be longer in nondipper patients than dipper ones (39.16+10.1 vs 30.11±10.0 and 44.69±11.74 vs 34.55+J1.85, respectively, p=0.001) CONCLUSION: QT dispersion is found to be prolonged in nondipper patients when compared with the dipper hypertensives. in our study left ventricular hypertrophy was not responsible for ventricular electrical instability. Measurement of QT dispersion and determining the absence of nocturnal blood pressure fail might be simple, non-invasive procedures to identify those hypertensives at increased risk of sudden death. These subset of patients are more likely to demonstrate severe ventricular dysrhytmias
