Abstract

Perioperative diuretics are commonly preferred drugs in the medical treatment of mitral valve diseases. However these drugsmay cause the syndrome of inappropriate secretion of ADH. At the other side, sitalopram, a kind of antidepressan drug which has a broad usage may produce clinical hyponatremia. We observed hyponatremia accompaying peripheral edema, pleural effusion, tremor, weakness and sleeplessness in a patient under diuretic and sitalopram therapy during perioperative mitral valve replacement. Symptoms are dramatically improved after interruption of sitalopram treatment. As a conclusion, etiological reasons of hyponatremia should be cleared and sitalopram with diuretic therapy should be consider as a etiological factor.

Özet

Mitral kapak hastalığının medikal tedavisinde, preoperatif ve postoperatif dönemde diüretikler sıklıkla kullanılmakta ve uygunsuz ADH salınımına neden olabilmektedir. Bununla birlikte son yıllarda depresyon tedavisinde sık kullanılan sitaloprama bağlı nadiren klinik hiponatremi tablosu gelişebilmektedir. Kliniğimizde mitral kapak replasmanı (MVR) uygulanan, preoperatif ve postoperatif dönemde diüretik ve sitalopram kullanan hastada; periferik ödem, plevral efüzyon, tremor, halsizlik ve huzursuz uyku semptomlarına eşlik eden hiponatremi tablosu gelişti. Hastada sitalopram tedavisi kesildikten sonra elektrolit bozukluğu nedeniyle uygulanan replasman tedavisine yanıt alınarak semptomlarda dramatik düzelme sağlandı. Açık kalp cerrahisi sonrası gelişen semptomatik hiponatremide etyoloji iyi belirlenmeli ve ayırıcı tanıda sitalopram tedavisi ile birlikte diüretiklerin kullanımı göz önünde bulundurulmalıdır.
