Abstract

Aim: Previous studies have suggested that presence of severe mitral regurgitation may have a protective effect against left atrial thrombus formation and peripheral embolic events. Mitral regurgitation is a frequent finding in patients with dilated cardiomyopathy. The aim of this study is to investigate the relation of mitral regurgitation to left atrial thrombus, spontaneous echo contrast (SEC) and left atrial appandage function, and development of thromboembolic events, and the relation of mitral regurgitation and mortality in patients with dilated cardiomyopathy during the follow-up period. Material and Method: The study group consisted of 93 patients (mean age 60±11 years) with the diagnosis of dilated cardiomyopathy. All patients were divided into 3 groups according to the severity of mitral regurgitation. Left atrial appandage function, presence of left atrial thrombi and SEC were evaluated with transesophageal echocardiography. The study group was followed for 28±10 months. Results: Mitral regurgitation was positively correlated with left atrial diameter (r=0.45, p<0.001), negatively correlated with left atrial emptying fraction (r=-0.27, p=0.016). There was no correlation between the severity of mitral regurgitation and both left atrial thrombi and SEC. Left atrial emptying velocity has a negative correlation with the severity of mitral regurgitation (r=-0.26, p=0.015) in the patient group. Twenty-three patients died during the follow-up period, there was a significant correlation between mortality and the grade of mitral regurgitation (p=0.02). History of thromboembolic events was present in 9 of the patients in the beginning of the study. New clinical thromboembolic events developed in another 9 patients of the study group. When patients with old and new thromboembolic events were evaluated together, 1 of them occurred in patients with severe regurgitation and all other events in mild-moderate grade of regurgitation (p=0.004). Conclusion: Severity of mitral regurgitation has no effect on the presence of left atrial thrombi and SEC, nevertheless cerebrovascular events show a tendency to decrease with the severity of regurgitation.
