Abstract

Objective: Elevation of neuron specific enolase in the absence of any clinically apparent stroke or transient ischemic attack, so called silent cere-bral infarcts may be associated with neuropsychiatric disorders and even increased mortality. We aimed to investigate elevation of neuron spe-cific enolase after cardiac procedures on the prediction of silent cerebral infarct to compare the effect of the arterial access site. Materrial and Method: Patients scheduled for coronary procedures from transfemoral and transradial access site were assessed for silent cerebral infarct. Study population consisted of two groups of patients. Group 1: transfemoral access (n:126), Group 2 transradial access (n:55). Neuron specific enolase elevation of greater than 0.12µg/L was considered as silent cerebral infarct. Results: Fourty-seven of 181 study patients (26%) had silent cerebral infarct after the procedure. Neuron specific enolase elevation was significantly more prevalant among patients with transradial access than transfemoral aproach (21% in group 1vs 36% in group 2, p=0.03). Multivariate analysis demonstrated history of smoking status (p<0.001), prior myocardial infarction (p = 0.001) and access site (p = 0.04) as independent predictors of silent cerebral infarct. Conclusion: Transradial catheterization is associated with a significant increase in silent cerebral infarct detected with neuron specific enolase compared to transfemoral catheterization. Increased diagnosis of silent cerebral infarcts might improve understanding of their relationship with invasive cardiac procedures.
