Abstract

AIM: Anterior myocardial infarction is frequently associated with left ventricular thrombus. Assessment of left ventricular thrombosis risk after myocardial infarction is important because of potential embolic sequelae and increased mortality. The aim of our study was to determine predictors of left ventricular thrombus formation after acute anterior wall myocardial infarction. MATERIAL AND METHODS: Two-dimensional and Doppler echocardiographic examination were performed in 109 consecutive patients (mean age: 58±12, 97 Male/ 12 Female) with first acute MI within 24 hours and day 3, 7, 15, and 30 after arrival the coronary care unit. Patients were divided into two groups according to left ventricular thrombus formation: Group l (n=21)patients with thrombus and Group 2 (n=88) patients without thrombus. RESULTS: LV thrombus was detected in 21 patients (group 1). Univariate regression analysis showed that patients with left ventricular thrombus had higher end-systolic volume (84±34 vs 59±28 ml, p< 0.05), and end-diastolic volume (134±40 vs 111±39, p<0.05), lower ejection fraction (38±12 vs 47±12, p< 0.05), a higher peak creatine kinase level (2321±1239 vs 1749±1212, p< 0.05), higher WMSI (1.9±0.28 vs 1.5±0.31, p< 0.05) than patients without left ventricular thrombus. According to the multiple logistic regression analysis, independent predictors of thrombus formation after anterior myocardial infarction were high end-sistolic volume (p<0.05). CONCLUSION: The current prevalence of LV thrombus in anterior wall AMI is lower than previously reported, possibly due to changes in AMI management. Our data demonstrate that the best predictor of left ventricular thrombus formation after acute anterior myocardial infarction is high initial end systolic volume.
