Abstract

AIM: Reoperations of coronary artery surgery still carries a higher mortality and morbidity rate compared to primary revascularization procedures. Coronary artery bypass reoperations on beating heart without the use of cardiopulmonary bypass is a new potential strategy that have been developed to decrease mortality and morbidity. MATERIAL AND METHODS: Between 1978-1999, 108 patients were reoperated for coronary artery revascularization, 31 (28.7%) cases on beating heart without the use of cardiopulmonary bypass and 77 (71.3%) cases under cardiopulmonary bypass group was 5 7.4± 7.3 years. RESULTS: Statistically significant difference was found between groups, favoring beating heart surgery when early postoperative findings taken into account, in low cardiac output, postoperative myocardial infarction and postoperative hospitalization period up to discharge. Operative mortality was not statistically different between groups and these figures for cardiopulmonary bypass and beating heart groups were 11/77 (14.3%), 1/31(3.23) respectively. CONCLUSION: Reoperations of coronary artery revascularization surgery without the use of cardiopulmonary bypass is more economic, less manipulations needing, free of the side effects of cardiopulmonary bypass, less time consuming surgical procedure compared to reoperations done under cardiopulmonary bypass and when performed with good technique and true indications, have mortality and morbidity advantages over the surgery performed under cardiopulmonary bypass.
