Abstract

AIM: The purpose of this study was to identify the effect of dobutamine on left atrial appendage (LAA) emptying velocity in chronic nonvalvular atrial fibrillation (AF) with relatively preserved left ventricular systolic function by transesophageal echocardiography (TEE). METHODS: The study group consisted of 21 patients (11 men and 10 women, aged 37 to years 62 [mean 51 ±7]) with chronic nonvalvular AF. After insertion of TEE probe, baseline heart rate and echocardiographic data were recorded on videotapes, then, intravenous dobutamine was infused with upward titration of the infusion rate at 3-minute intervals according to the following schedule: 2.5, 5, 7.5, and 10 µgr/kg/min. Transesophageal Doppler and two-dimensional recordings were done continuously during the infusion period. RESULTS: Significant (p<0.001) increases in LAA emptying velocities (from 19±3 cm/sec to 27± 2 cm/sec) occurred during the maximal dobutamine infusions. Left ventricular ejection fraction rose from 56±3% prior to infusions to 59±4% during the maximal dobutamine infusions (p<0.001). Dobutamine significantly increased heart rate (from 77± 5 to 94± 6 beats/min; p<0.001). Before the dobutamine infusion, left atrial spontaneous echo contrast (SEC) were observed in 12 patients. With intravenous dobutamine infusion, intensity of LAA SEC decreased in only two patients. CONCLUSION: We concluded that the f) adrenergic agonist dobutamine could improve the LAA emptying velocity in patients with chronic nonvalvular AF with relatively preserved left ventricular systolic function, however, awaits further studies.
