Abstract

Aim: in previous studies, terminal QRS distortion in ECG of patients with acute anterior myocardial infarction (MI) in emergency department has effect on early phase risk factors and affects the in hospital mortality, reinfarction, heart failure and ejection fraction. in our study we investigated the effect of abnormality in last part of QRS on short term prognosis. Material and Method: One hundred tvventy öne patients with first MI were included in the study. Patients were divided into 2 group according to last part of QRS complex as with QRS distortion (QRS +) and vvithout QRS distortion (QRS-). Results: There was no significant difference betvveen the groups regarding age, sek, risk factors and preinfarction angina. Anterior infarction was more commonly seen and infarction region assessed by CK MB was larger in patients with QRS distortion. Left ventricle ejection fraction was lovver in patients with QRS distortion. in hospital complication rate was also higher in the distortion group. Conclusion: in patients presented with QRS distortion in first ECG left ventricle functions were worse and the probability of complication development was higher. These results may be explained by higher rate of LAD involvement and larger tissue injury.
