Abstract

Aim: To evaluate effort capacity, the left and right ventricle functions in mitral regurgitation (MR) patients with and without chordal rupture(CR). Material and Method: 62 MR patients,35 with CR and 27 without CR, were included in the study.(In CR group,14 were females and 21 were male. The mean age was 61 ± 17 years . In the non-CR group,16 were females and 11 were males. Their mean age was 58 ± 14 years.Left ventricle ejection fraction (LVEF) was calculated using modified Simpson’s method from the apical 4- chamber view. Tricuspid valve annular plane systolic excursion (TAPSE) was used to evaluate the right ventricle function. Results: There was a significant difference between the two groups in left ventricle ejection fraction (MR with CR: 65±10 %, MR without CR: 55±13%, p= 0.0001), left venricle end-diastolic diameter (MR with CR: 3.5±0.9cm, MR without CR: 4.0±0.8cm, p = 0, 03) and ERO area (MR with CR: 71±19,MR without CR: 65±, 17, p = 0,039). Although there was no significant difference between the TAPSE values of the two groups, patients in CR groups had higher values. Difference in NYHA class was not significant between the two groups but MR without CR group had higher class of NYHA. Conclusion: There was a significant difference in left ventricle systolic functions in patients with and without CR while right ventricle functions are not significantly different.

Özet

Amaç: Bu çalışmada korda rüptürüne (KR) bağlı mitral yetersizliği (MY) ile diğer sebeplere bağlı MY’li hastaların efor kapasitesi, sol ventrikül ve sağ ventrikül sistolik fonksiyonlarını değerlendirmeyi amaçladık. Gereç ve Yöntem: Çalışmaya 35 korda rüptürüne bağlı mitral yetersizliği olan ile 27 diğer sebeplere bağlı mitral yetersizliği olan toplam 62 hasta dahil edildi. (KR olan MY’li hastalar 14 kadın, 21 erkek, yaş ortalaması 61 ± 17 yıl; diğer sebeplere bağlı MY’li hastalar 16 kadın, 11 erkek, yaş ortalaması 58 ± 14 yıl). Sol ventrikül ejeksiyon fraksiyonu (SVEF) apikal dört boşluktan modifiye Simpson’s metodu ile hesaplandı. Sağ ventrikül sistolik fonksiyonunu belirlemek için triküspit kapağın anüler plan sistolik hareketi (TAPSH) M-mod ekokardiyografik yöntemle değerlendirildi. Bulgular: İki gurubun sol ventrikül ejeksiyon fraksiyonu (KR’ne bağlı MY:65±10; diğer sebeplere bağlı MY: 55±13, p = 0.001), sol ventrikül sistol sonu çapı (KR’ne bağlı MY: 3.5±0.9; diğer sebeplere bağlı MY: 4.0±0.8, p = 0.03) ve efektif regürjitan orifis alanı (KR’ne bağlı MY: 71±19; diğer sebeplere bağlı MY: 65±, 17, p = 0.039) arasında anlamlı fark bulundu.
