Abstract

AIM: It's claimed that, stenting after debulking can reduce the restenosis in the case of high plaque volume. MATERIAL AND METHODS: 4 females, 40 males patients (n=44) with mean ages of 54.86±24.2 were included. 10 of lesion were total. Average MLD was 0.324 ± 0.055 before ELCA. 1.7C catheter was used for 28 patients, 1.4C catheter for 5 patients, 1.7E catheter for 6 patients and 1.7C catheter was used after 1.4C catheter application in 5 patients. Mean lasing time and range of lasing energy frequency and pulsation were 14 ± 4 and 45/25-50/30 respectively. Average MLD was 1.815 ± 0.1 mm after ELCA (p<0.001). Stents were implanted to all patients. Mean stent length was 14.27 ± 3.6 mm. Average MLD was 3.317 ± 0.06 mm after stent implantation. RESULTS: The patients were followed for 9 months. Non-Q MI was seen in two patients during this period. Control angiograms of 42 patients (except 2 non-Q MI patients) were done. Restenosis was observed in 4 cases(9.5 %). 9 months major adverse cardiac events (MACE) was developed in 6 cases(14%). 9 months cardiac free survival rate of patients was 86 %. Restenosis and 9 months MACE ratio were significantly low in spite of the total occlusion in 23 % of cases. CONCLUSION: It seems that stent implantation after ELCA reduced the angiographic restenosis and cardiac events in selected cases.

Özet

AMAÇ: Plak volümünün yüksek olduğu lezyonlarda, debulking sonrası stent uygulamasının stent restenozunu azaltabileceği iddia edilmiştir. GEREÇ VE YÖNTEM: Çalışmamızda, yaş ortalaması 54,86 ± 2,42 olan, 4 bayan 40 erkek (n=44) hasta alındı. Lezyonların 10'u (% 23) total oklüzyondu. Lezyonların damarlara göre dağılımı 14 (% 32) sol ön inen arter, 5 (% 11) sol sirkumfleks, 25 (% 57) sağ koroner arter yerleşimli idi. Excimer laser coronary angioplasty (ELCA) öncesi ortalama minimal luminal diameter (MLD) 0,324 ± 0,055 mm idi. ELCA işlemi için, 28 vakada 1.7 C Vitesse catheter, 5 vakada 1.4 C Vitesse catheter, 5 vakada 1.4 sonrası 1.7 C Vitesse catheter, 6 vakada ise 1.7 E Vitesse catheter kullanıldı. Ortalama laser sayısı 14 ± 4, laser enerji frekans ve pulsasyon aralığı 45/25-50/30 idi. ELCA sonrası MLD 1,815 ± 0,1 mm idi. Aradaki istatistiksel fark p<0.001 idi. Tüm vakalarda ELCA sonrası perkutan transluminal koroner anjioplasti(PTKA) yapılmaksızın stent implantasyonu 12-14 atm. arasında yapıldı. Ortalama stent uzunluğu 14,27 ± 3.6 mm idi. Stent sonrası ortalama MLD 3,317 ± 0,06 mm idi. Hastalar 9 ay takip edildi. İşlem sırasında, ELCA sonrası 5 vakada Tip B diseksiyon gelişti. Hastane içi non-Q, Q miyokard infarktüsü, acil KABG hiçbir olguda gelişmedi ve re-revaskülarizasyona gerek duyulmadı. BULGULAR: Tüm hastalar 2. günde taburcu edildi. Takip süresinde 2 vakada (% 4,5 ) non-Q miyokard infarktüsü gelişti. Koroner anjiografi 42 vakada (2 non-Q vaka hariç) uygulandı. Dört vakada (% 9,5) restenoz saptandı. Dokuz aylık MACE(ölüm, myokard enfarktüsü, KABG, tekrar perkutan girişim) 6 vakada ( % 14) gelişti. Hastaların 9 aylık kardiyak olaysız yaşam oranı % 86 idi. SONUÇ: Sonuç olarak, vakaların % 23'ünde total tıkalı olmasına rağmen, restenoz ve 9 aylık MACE oranı önemli ölçüde düşüktü. Seçilmiş vakalarda ELCA sonrası stent uygulaması anjiogafik restenozu ve kardiyak olayların oranını azaltıyor görünmektedir.
