Abstract

Aim: Reoperative cardiac surgery continues to be a challenge to cardiac surgeons. There are many problems in reoperative cardiac surgery compared with the previous cardiac surgery. We share our surgical experiences in reoperative cardiac surgery over a period of 17 years. Material and Methods: From January 1989 to August 2005, a total of 10,789 cardiac operations were performed in our department of cardiovascular surgery. 185 out of them were reoperative surgeries. Preoperative conditions of patients, their primary operations, reoperation interval, results of operation and postoperative complications were evaluated retrospectively using patient files, computer recordings and surgery reports in regard to the technique we used, and consequences of that technique. Results: Of those patients, 125 (67.56%) were males, and 60 (32.43%) were females. The mean age was (59.05±13.75). The interval between primary operation and reoperation was calculated as 11±6 years. 17 patients (9.18 %) were re-explored due to hemorrhage. 80 of those patients (43.24%) needed high-dose inotropic and 62 others (33.51%) needed intra-aortic balloon support after undergoing reoperation. Postoperative cardiac block developed in 27 patients (14.59%). Almost all patients were discharged from the hospital under normal circumstances. 41 patients (22.16%) had to stay at hospital more than 14 days due to curable complications [prolonged intubation (>8 h), bedsores, infection in sternum wound and cardiac block etc.] or hemodynamic instability. Mortality rate was set as nine patients (4.86%). Conclusion: In experienced hands, reoperative cardiac surgery could be conducted safely with a low rate of morbidity and mortality. Reduction of operative risks must be attributed to better myocardial protection, better preoperative patient management and operative management. Elective repaet cardiac reoperations give the best results.

Özet

Amaç: Kardiyak reoperasyon cerrahisi, kalp cerrahlarına sıkıntı oluşturmaya devam etmektedir. İlk uygulanan cerrahiyle kıyaslandığında, reopere kalp cerrahisinde pek çok problem söz konusudur. Bu çalışmada, reopere kalp cerrahisindeki on yedi yıllık deneyimlerimizi paylaşıyoruz. Bu vakaların 185’i reopere kalp cerrahisiydi. Hastaların preoperatif durumları, primer operasyonları, reoperasyon aralığı, operasyon ve operasyon sonrası komplikasyonları retrospektif olarak hasta dosyaları, operasyon raporları ve bilgisayar kayıt sistemi üzerinden araştırıldı. Bulgular: Hastaların 125’ i erkek (%67.56), 60’ı kadın (%32.43) olarak belirlendi.
