Abstract

AIM: Analysis of the efficacy of PTCA at in stent restenosis (ISR) in the light of the long-term angiographic and clinical data MATERIAL AND METHODS: The clinical data of the 53 patients with proven ISR was analyzed and quantitative angiography was performed before and immediately after the PTCA and at the 6 month. Earlier angiographic evaluations were done if necessary. RESULTS: Results: None of the patients had acute MI and procedure related requirement of urgent bypass surgery. In the follow-up period of 5.9±4.3 months 26.4% of the patients were asymptomatic, which only one had restenosis. During the follow up period, myocardial infarction, serious arrhythmia, heart failure developed in three (5.6%), five (9.4%) and two (3.7%) patients, respectively. Five patients (9.4%) underwent to coronary bypass grafting and two (3.7%) patients died . Angiographic recurrent ISR was found 58.4%. The minimal luminal diameter and having multiple stents before the procedure were correlated to recurrent ISR after PTCA of ISR. Diabetes mellitus and hypertension were the risk factors for the clinical adverse events. The incidence of recurrent restenosis in patients with diffuse type ISR was slightly higher than in patients with focal ISR but statistical significance was not found. The types of rerestenosis were not correlated with the types of ISR before PTCA. CONCLUSION: Though PTCA is an easy, practical and reliable procedure for the correction of ISR, the recurrence rate is high. The type of the ISR is not a prognostic marker to foresee the nature of recurrence after PTCA.
