Abstract

Aim: Physiologic changes during pregnancy may cause severe problems on the mother after delivery. Postpartum infective endocarditis (PPE) is one of these condition and seen quite rarely. Material and Method: in this study, we analyzed 6 PPE cases whose we diagnosed within last seven years. Diagnosis were made based on Duke criteria. The patiens were 29.0$\pm$2.6 years old, and four of them had rheumatic valve disease (öne had biologic valve prosthesis), while two of them had no any diagnosed valve disease before pregnancy. Signs of infective endocarditis were noticed 18.8±11.2 days after delivery. Results: in five patients mitral and in öne patient aortic valve were diseased. Causative agents were Staphylococcus aureus in two of the cases, and viridans streptococcus in öne of the case. in two cases no any agent was detected. 4.0$\pm$1.1 week-antibiotherapy was applied to the patients. Complete cure were achieved by medical therapy in two patients, while valvular replacement was required in four patients. AH of the cases were discharged with cure. Conclusion: Hormonal changes during pregnancy period creates tolerance to the fetal antigens by suppressing cellular immunity. This causes a vveaker resistance to the infections. When immun system is weak, microorganisms may cause endocarditis due to a possible bacteriemia during delivery. We think it vvould be useful to do antimicrobial prophylaxia during delivery routinely in the cases especially who have predisposing factors like valvular heart disease. Also, these patients should be follovved closely at postpartum period for endocarditis risk.
