Abstract

AIM: The purpose of our study was to investigate coronary blood flow properties in patients with isolated coronary artery ectasia (CAE) who had exercise-induced myocardial ischemia. BACKGROUND: Metabolic evidences of myocardial ischemia were previously documented in patients with CAE. Although it has been postulated that altered coronary blood flow in CAE predisposes patients to the development of myocardial ischemia, this hypothesis has not been systematically tested before. MATERIAL AND METHODS: CAE was defined as luminal dilatation of at least 1.5 folds of adjacent normal coronary artery segment, or the diameter of corresponding coronary artery of the control group if there was no normal segment. Forty-one patients with CAE without associated coronary artery stenosis (isolated CAE) were derived from 3815 coronary angiographies between 1998 and 2000. Twenty-four patients were excluded from the study because of previous myocardial infarction, uncontrolled hypertension, left ventricular hypertrophy and a high resting heart rate. Seventeen patients with isolated CAE who met the inclusion criteria were enrolled into the study (CAE group). Age and sex matched control group (n=20) was comprised of patients with normal epicardial coronary arteries. Coronary blood flow velocities were obtained invasively by means of a 0.014 inch-diameter Doppler flow wire. Coronary flow reserve (CFR) was measured by administration of intracoronary papaverine as the hyperemic stimulus. RESULTS: All patients with isolated CAE developed myocardial ischemia on ergometry with ST depressions of 0.22±0.03 mV, whereas none of the control patients met electrocardiographic criteria of myocardial ischemia. Angiographic signs of impaired coronary blood flow were detected in 16 patients with CAE, that were defined as slow flow, segmental backflow and local deposition of dye. Baseline average peak velocities (APV) of coronary blood flow was lower in the CAE group than in the control group with a borderline statistical significance (11.9±5.0 cm/sec vs. 15.9±5.6 cm/sec, respectively, p=0.08). Peak hyperemic APV's of coronary blood flow were also lower in the CAE group than in the control group (17.5±7.4 cm/sec vs. 41.5±12.6 cm/sec, respectively, p<0.001). Mean CFR of the CAE group was significantly reduced as compared to the control group (1.51±0.31 vs. 2.67±0.52, respectively, p<0.001). In the CAE group, CFR was correlated to the exercise capacity defined as metabolic equivalent (MET) on ergometry (R= 0.8, p=0.001). Additionally, there was a moderate inverse association between CFR and the diameter of ectatic coronary artery in which CFR was measured (R= -0.6, p<0.02). CONCLUSIONS: Reduced coronary flow reserve is underlying cause of exercise-induced myocardial ischemia in patients with nonobtructive coronary artery ectasia.
