Abstract

AIM: We investigated the association between the time of onset of atrial fibrillation (AF) and postinfarction complications and prognosis. MATERIAL AND METHODS: We evaluated 525 consecutive patients admitted with acute myocardial infarction (AMI) in our coronary care unit between 1997 and 2000. First, the patients were categorized on the basis of the presence of sinus rhythm (Group I: n=300; 164 male, mean age: 57±10) and atrial fibrillation ( Group II: n=225; 169 male, mean age: 61±10). Then, group II patients were divided into three subgroups: AF group 1 (n=40), who developed AF within 24 hours after the onset of MI. AF group 2 (n=58), who developed AF after 24 hours after the onset of AMI. AF group 3 (n=127), who developed AF before the onset of AMI. In group I and II, we compared postinfarction complications, coronary angiography findings and 1year mortality rate. Futhermore, these endpoints were compared in AF subgroups. RESULTS: The localisation of AMI, prevelance of cardiogenic shock, pericarditis, complete AV block, and in-hospital mortality was similar in patients between with and without AF (p>0,05). In group I patients, prevelance of ventricular tachycardia (VT) and single vessel disease in coronary angiography was significantly more frequent (p<0.05 and p<0.001 respectively) and ejection fraction (EF) was higher (p<0,001) than group II. The frequencies of heart failure, 3 vessel coronary artery disease, thromboembolism and 1-year mortality were higher for all 3 AF groups than the sinus group (p<0.01, p<0.001, p<0.01, p<0.05 respectively). Among AF subgroups, there was no significant difference in prevelance of pericarditis, thromboembolism, complete AV block, and 2 or 3 vessel coronary artery disease. Altough the EF was higher in AF1 group ( between AF1 and AF2, p<0.05; between AF1 and AF3, p<0,001), the prevelance of cardiogenic shock and ventricular septal rupture (VSD), VT and ventricular fibrillation (VF), in-hospital mortality and one vessel coronary artery disease was significantly more frequent (p<0,001, p<0,001, p<0,01, p<0,05 respectively).1year mortality did not differ significantly between subgroups. CONCLUSION: Patients with AMI, who develop AF in the first 24 hours more often had cardiogenic shock, VT,VF,VSD and higher in-hospital mortality. More aggressive treatment strategies in this subgroup may be warrented.

Özet

AMAÇ: Akut miyokard infarktüsünde Atrial fibrilasyon başlama zamanının, klinik bulgular, infarkt sonrası komplikasyonlar ve prognoz üzerine etkisini araştırmak GEREÇ ve YÖNTEM: 1997-2000 yılları arasında akut Mİ tanısı ile koroner yoğun bakım ünitesine yatırılan 525 hasta araştırılmaya dahil edilerek 1 yıl takip edildi. Hastalar 2 gruba ayrıldı: Grup I: Sinüs ritmindeki AMİ vakaları (n=300, 164 Erkek, ortalama yaş:57±10) ve Grup II: AF gelişen AMİ vakaları (n=225, 169 Erkek, ortalama yaş:61±10).
