Abstract

Aim: A lot of factors are proposed to interfere with coronary collateral circulation (CCC) development vvhich is thought to have important role in coronary perfusion. We aimed to determine the effect of occluded coronary artery on the degree of CCC development. Materials and Methods: Four hundred and seventy two patients (219 male, 253 female) with total occlusions at proximal ör mid portions of öne of the coronary arteries were included. Collateral circulation was graded according to Rentrop classification. Patients who have grade O and 1 collaterals were grouped as poor collaterals whereas with grade 2 and 3 were grouped as good collaterals. Clinical and angiographic factors thought to effect CCC development weere analyzed with multivariate logistic regression analysis. Results: The presence of angina, well preserved left ventricular function and coronary occlusion localization at the right coronary artery (RCA) were found to be related with good collateral development whereas diabetes mellitus was found to be related with poor collateral development (p<0.05). The occluded vessel was left anterior descending artery (LAD) in 212, RCA in 193, and left circumflex artery (Cx) in 67 patients. The RCA occlusion was found to be an independent and more powerful predictor of good collateral development than LAD and Cx occlusions (OR: 3.08, p: 0.02; OR: 5.75, p: 0.0001 respectively). Conclusion: We may conclude that RCA occlusion is an important factor effecting collateral development and well developed collaterals preserves left ventricular functions. Also, the presence of angina pectoris is related with good CCC development.
