Abstract

AIM: Cyclosporine-based triple therapy has been shown to favorably influence cardiac transplant recipients. It is very important to monitor rejection and ensure the adequacy of immunosuppressive therapy. MATERIAL AND METHODS: All of the 18 patients received triple drug immunosuppression (Cyclosporin A, Azathioprine, Prednisone). The diagnosis of acute rejection was established by endomyocardial biopsy according to criteria defined by the International Society of Heart and Lung Transplantation. One hundred-fifty eight biopsies were performed in sixteen patients apart from the 2 patients who died in the perioperative period. RESULTS: No rejection was identified in 82 biopsies, grade IA in 42, grade IB in 8, grade II in 9, grade IIIA in 12 and grade IIIB in 1. Acute rejection (grade IIIA or higher) was treated with 1 gr methylprednisolone intravenously daily for 3 days. In one patient, resistant rejection was managed with rabbit antithymocyte globulin (200 mg/day for 5 days). In two patients, who had taken pulse corticosteroid therapy for grade IIIA acute rejection, cytomegalovirus reactivation was followed by fatal pneumonia (aspergillus in one, staphylococcus in the other). Rabbit antithymocyte globulin was given to 2 patients because of acute renal failure requiring dialysis. In one of them, aspergillus pneumonia developed and the patient died. In 3 patients, cyclosporin was replaced with tacrolimus. CONCLUSION: The introduction of new immunosuppressive agents and repeated endomyocardial biopsies after cardiac transplantation resulted in a decrease in rejection incidence. On the other hand chronic immunosuppression causes several untoward effects and increased susceptibility to transplant-related infections.

On altı olguda toplam 158 biopsi yapılmış, 2 olgu erken dönemde kaybedildiği için biopsi yapılmamıştır.
