Abstract

AIM: 25 patients with mediastinitis were retrospectively evaluated for the preoperative, operative and postoperative risk factors for mediastinitis after open heart surgery during ten year period. MATERIAL AND METHODS: 25 of 3063 patients with open heart surgery found to have mediastinitis between the dates April 1991 and March 2001. 223 patients without mediastinitis were selected randomly as a control group. Potential risk factors were compared with the two groups. RESULTS: The incidence of mediastinitis among all patients is 0,81% and 1,37% for coronary surgery and 0,48% for valvular surgery. 22 (88%) of 25 patients were reoperated because of sternal dehisence and wound discharge with a mean of 14,46± 13,4 (SD) (7-60) days after the surgery. In mediastinitis group, as predictive factors diabetes mellitus (p=0,018), smoking (p=0,001), IMA usage (p=0,001), total CPB time (p=0,009), cross clemp time (p=0,013), mechanical ventilation more than 24 hours (p=0,011), long stay in the intensive care unit (p=0,003) is found to be significant than control group. Multipl logistic regression analysis show that diabetes mellitus and long cardiopulmonary bypass time are independent predictor factors for mediastinitis. The predictor power of other risk factors are found to have less significant power. CONCLUSION: Operating room is the most common infection source. Mediastinitis can be prevented with decent timing of prophylacise antibiotic usage and preperation of patient for operation, obeying of surgical team to the antiseptic rules, maximal bleeding control as a surgical technique, selection of proper sternum closure technique.
