Abstract

AIM: There are conflicting study results about the effects of estrogen (ERT) and hormone replacement therapy (HRT) on left ventricular diastolic functions. In these studies, the pseudonormal type of diastolic dysfunction, which may cause this argument, was not taken into account. In the present study, we aimed to overcome this limitation by using E/Em (the ratio between early diastolic mitral E wave velocity and early diastolic mitral annular tissue Doppler velocity), which is related to left atrial pressure, and to investigate the effects of hormonal therapy on left ventricular diastolic function. MATERIAL AND METHODS: The two study groups were composed of 20 postmenopausal women having HRT and 27 postmenopausal women taking ERT. Each group was evaluated for left ventricular diastolic function at both the pre-treatment stage and after 12 weeks of hormonal therapy. In regard with diastolic function, the ratio between maximal early and late mitral flow velocity (E/A), deceleration time of mitral early E wave, isovolumetric relaxation time and mitral annular early diastolic tissue Doppler velocity (Em) were measured. RESULTS: Heart rate (83±12' vs. 78±12 beat/min, p=0.01) and mitral annular E/Em ratio reduced (7.18±3.09 vs. 5.56±2.15, p=0.001), while the number of patients with normal diastolic functions increased (%52 vs %85, p=0.006) with ERT. HRT was observed to reduce heart rate (79±12 vs. 75±12, p=0.026) and mitral E/Em ratio (7.61±3.31 vs. 5.81±2.31, p=.003), while the number of patients with normal diastolic function increased with this therapy (from 65% to 85%, p=0.003). CONCLUSION: Both ERT and HRT have improving effects on left ventricular diastolic function.
