Abstract

Aim: It is known that platelet aggregation following plaque rupture is the main step in thrombus formation in the coronary artery. The alterations in platelet volume and activity directly influence on intracoronary thrombus formation and the extent of myocardial infarction. In the present study the effect of mean platelet volume (MPV) on one year mortality and on the need for revascularization is investigated in patients presenting with myocardial infarction. Materials and Methods: 87 patients with acute myocardial infarction and 32 control patients were enrolled in the study. Venous blood samples for determining MPV were taken from patients on the first day of hospitalization period. Then patients were followed for one year mortality and the need for revascularization. The relation between one year mortality and the need for revascularization and mean platelet volume on arrival was evaluated. Results: Occurring on the first week for 14 patients; totally 22 patients were died during one year follow up. Platelet count and MPV in surviving and non suviving patients were 265.000$\pm$112.000/mm3, 9.6$\pm$0.8 fl and 264.000$\pm$133.000/mm3, 10.6$\pm$0.9 fl respectively. While no statistical significance were detected between platelet counts; MPV was significantly greater in non surviving patients (p<0.001). Additionaly the need for coronary angioplasty and coronary bypass surgery were more frequent in the patients with large platelet volume (p<0.001). Conclusion: As a conclusion it was established that mortality and the need for revascularization were more frequent in acute myocardial infarction patients with greater MPV and also more intensive care is needed in follow up and treatment for these patients.
