Abstract

previously healthy 31 year old male presented with syncope. The patient was monitorized and a VF episode was documented during the recurrence of syncope. The ECG revealed down slopping ST segment elevation in leads V1 through V2 and right bundle branch block and because of typical ECG findings, Brugada syndrome was suspected. Echocardiography and coronary angiography demonstrated normal left ventricular function and normal coronary arteries. During the follow-up resting ECG was normal and became abnormal following administration of intravenous propafenon (1 mg/kg body weight, 10 mg/min). ICD was implanted due to inducibility of VT at programmed electrical stimulation. The event-free period between ICD implantation and the first spontaneous recurrence of VF was five months. The patient received two shocks during syncope and interrogation of the device revealed detection of VF. The patient who had typical features of Brugada syndrome and the other patient who have had Brugada ECG pattern are presented.

Özet

Senkop nedeniyle özel bir sağlık merkeze başvuran 31 yaşındaki erkek hasta, tekrar senkop meydana gelmesi üzerine monitorize edilmiş ve ventriküler fibrilasyon (VF) saptanması üzerine defibrilasyon uygulanarak sinüs ritmi sağlanmış. Hasta, ileri tetkik ve tedavi amacıyla tarafımıza refere edildi. Elektrokardiyografisinde (EKG) V1-2'de "down-slopping" ST segment elevasyonu ve sağ dal bloğu paterni saptanan hastanın Brugada sendromu olabileceği düşünüldü. Hastanın ekokardiyografisi ve koroner anjiyografisi normal saptandı. Takiplerinde ST segment elevasyonu ve dal bloğu kaybolan hastada propafenon ile provokasyon testi yapıldığında tekrar sağ dal bloğu ve ST segment elevasyonunun yeniden ortaya çıktığı görüldü. Elektrofizyolojik çalışmada ventriküler taşikardi (VT) indüklenen hastaya implante edilebilen kardiyoverter-defibrilatör (ICD) implante edildi. Takiplerinde VF nedeniyle ICD tarafından elektroşok uygulandı. Brugada sendromunun tipik özelliklerini taşıyan bu olgu ile birlikte çeşitli testler yapılan Brugada sendromunun EKG bulguları izlenen asemptomatik hasta sunuldu.
