Abstract

A 66-year-old Caucasian woman with the diagnosis of atypical pneumonia was referred to echocardiography laboratory for evaluation of heart murmur. On transthoracic echocardiogram, apical-fourchamber view demonstrated apically displaced septal and anterior tricuspid leaflets suggesting Ebstein’s anomaly. The tricuspid valve was thick and elongated with moderate to severe tricuspid regurgitation. Functional right ventricular cavity was diminished whereas right atrium was enlarged. She also had preaxial polydactyly on the right thumb. The patient was hemodynamically stable with no heart failure symptoms or arrhythmias and no surgical intervention was performed. This case highlights that Ebstein’s anomaly of the tricuspid valve has a variable natural history. Survival to sixth decade without any symptoms is extremely rare. The coexistence with polydactyly can be significant or coincidental.

Özet

Atipik pnömoni sırasında tesadüfen tespit edilen kardiyak üfürüm nedeniyle ekokardiyografi laboratuarına yönlendirilen 66 yaşında kadın hastada transtorasik ekokardiyografi ile triküspit kapağın septal ve ön yapraklarının apikale doğru yer değiştirdiği saptandı. Triküspit kapak kalın ve uzundu. Bu durum orta-ağır derecede triküspit geri akımına neden olmaktaydı. Fonksiyonel sağ ventriküler hacmin küçülmesine karşın sağ atriyum genişlemişti. Kalp yetmezliği ya da aritmi bulguları yoktu. Oldukça nadir olmasına karşın Ebstein anomalisi altıncı dekata kadar semptom vermeden kalabilir. Olgumuzda ileri yaşta tesadüfen tanı konmuştur.
