Abstract

Aim: This prospective, controlled, clinical study was planned to determine the effect of pretreatment with atorvastatin on cardiac morbidity and mortality after beating heart coronary artery bypass surgery. Material and Method: Patients (n=30) with coronary artery disease who required beating heart coronary artery bypass grafting were allocated into two groups. Patients in group 1 (n=15) received atorvastatin before surgery. Patients in group 2 (n=15) were control group and did not receive statin. Measurements of circulating creatine kinase MB (CK-MB) and Troponin I (TnI) concentrations were obtained before surgery and at 4, 12, 24, and 48 hours postoperatively. Preoperative risk profiles, operative variables, mortality, morbidity, incidence of postoperative atrial fibrillation(AF) and data associated with operation were recorded in all patients. Results: Preoperative risk profiles and operative variables were statistically similar when the groups were compared. The concentration of circulating CK-MB and Tn I significantly increased over time after off - pump coronary artery surgery, with no significant intergroup differences. Median occlusion time per anastomosis (7;5-12 minutes in group 1 versus 6.8; 5-11 minutes in group 2) was not significantly different between groups. One patient had atrial fibrillation in group 1, while 2 patients had AF in group 2, with no significant intergroup differences. There was no hospital mortality, no neurological complication, pulmonary insufficiency or myocardial infarction in the atorvastatin and control groups. Conclusion: This study shows that pretreatment with atorvastatin did not significantly effect myocardial damage during off-pump coronary surgery when the local ischemic time was limited.

Özet

Amaç: Preoperatif atorvastatin kullanımının çalışan kalpte koroner cerrahisi sonrası kardiyak morbidite ve mortalite üzerine etkilerini araştırmak üzere ileri dönük, kontrollü klinik çalışma planlandı. Gereç ve Yöntemler: Çalışan kalpte koroner cerrahisi yapılacak toplam 30 koroner arter hastası 2 gruba ayrıldı. Grup 1 (n=15) hastalar cerrahi öncesi atorvastatin kullanan, Grup 2 (n=15)’ ise statin tedavisi almayan hastalardı ve kontrol grubu olarak çalışmaya alın- dılar. Tüm hastalarda kreatin kinaz MB (CK-MB), Troponin I (Tn I) ölçümleri cerrahi öncesi, postoperatif 4. saatte, 12. saatte, 24. saatte ve 48. saatte yapıldı.
