Abstract

Aim: Several mechanisms including tethering of mitral leaflets, mitral annular dilatation, papillary muscle dysfunction and altered left ventricular function contributes to the development of mitral regurgitation (MR) after myocardial infarction (MI). In this study we aimed to evaluate the synchronicity of SV in patients with ischemic MR. Material and Method: We prospectively enrolled consecutive 24 acute ST elevation MI patients with functional moderate or severe MR, characterised by regurgitation despite structurally normal valve. Age and gender matched 23 acute ST elevation MI patients without MR were selected as controls. Tissue synchronization imaging (TSI) was performed in all study population. LV asynchrony was identified on TSI images and the time to regional peak systolic velocity (Ts) in LV was measured by the six-basal-six-midsegmental model. Results: The standard deviation of Ts of 12 LV segments (55±16 vs. 32±15 ms, p=0.0001) and 6 basal LV segments (43±22 vs. 27±15 ms, p=0.009), maximal difference in Ts between any 2 of the 12 LV segments (162±39 vs. 105±58 ms, p=0.0001) and between any 2 of the 6 basal LV segments (111±50 vs 74±46 ms, p=0.014) were prolonged in patients with MR. Ts-septal-lateral delay (46±40 vs. 31±43 ms, p=0.23) was similar between the groups. However Ts-septal-posterior delay (55±38 vs. 32±31 ms, p=0.035) was significantly higher in patients with MR than the controls. All asynchrony indices were significantly corelated with MR jet area except Tsseptal lateral delay. Conclusion: LV synhronicity indices were prolonged in patients with ischemic MR.

Sonuç : MI sonrası iskemik MY gelişen hastalarda SV senkronisite parametreleri uzamıştır.
