Abstract

Objective: Digoxin is one of the commonly used cardiac drugs in our country. It was shown that, digoxin had been used most common in Turkey among European countries, according to the studies. The aim of the study was to evaluate whether digoxin was used or not according to indications of the current guidelines, and to investigate the relationship between digoxin oral taking time and blood digoxin levels and its clinical importance, and the other factors affecting the blood digoxin level. Material and Method: Patients, who were admitted to Ankara University School of Medicine-Department of Cardiology, and digoxin level were controlled, enrolled to our study. Patients demographic characteristics, indications for use of digoxin, accompanying diseases, laboratory findings, creatinine clearance, electrocardiography and echocardiography findings, medications, digoxin usage, and the blood sample taking hour after digoxin taking were recorded. Results: A significant difference between serum levels of digoxin and blood sample taking hour after pill swallowing was not detected (r=0.056, p=0.586). It was demonstrated modest relationship between serum digoxin level and the time of worked of digoxin level (r=-0.38, p=<0.01). The factors affecting digoxin level statistically significant difference were found as: a weekly dose of digoxin (B=0.221, p=0.001), and creatinine clearance (B=-0.213, p=0.001). A positive and a weak to moderate statistically significant correlation was observed between digoxin dose difference (between the recommended doses of digoxin based on nomogram and the digoxin dosage used by the patient) and the serum digoxin level (r=0.36; p=0.007). As we investigate the relationship between the drugs that affect the level of serum digoxin and digoxin level, only warfarin was shown a weak-to-moderate positive correlation with digoxin blood level (r=0.28; p=0.05). Conclusion: The relationship between digoxin oral taken time and blood levels were investigated due to the lack of standardization in determining the level of digoxin and a significant statistical difference was found. According to the available data, digoxin should be taken as a single dose in the evening hours and the measurement of digoxin level on minimum 6 hours and optimal 12 hours after tak- ing digoxin dosage may be recommended in clinical practice. It is recommended that regular control of digoxin levels in patients using warfarin.
