Abstract

Domperidone is widely used for pharmacological treatment of gastroparesis, gastroesophageal reflux diesase and functional dyspepsia. In in-vitro studies it has been observed that domperidon prolongs repolarizaton and action potential duratiın by binding to cardiar hERG (human ether-a-go-go-related gene) protein, which is a subunit of late rapid repolarizing potassium channel (Ikr). Various case reports and retrospective studies have reported prolongation of QT interval with chronic domperidon use. Similarly, risk of sudden cardiac death and ventricular arrythmias has been reported to be increased, although mainly by meta-analysis of retrospective data. In this article we review current data regarding cardiovascular effects of chronic domperidon use.

Özet

Domperidon gastroparezi, gastroözefageal reflü ve fonksiyonel dispepsi gibi birçok intestinal motilite bozukluklarının farmakolojik tedavisinde kullanılmaktadır. in vitro çalışmalarda domperidonun geç hızlı repolarizasyonu sağlayan potasyum kanallarının (IKr) subünitesi olan kardiyak hERG (human ether-a-go-go-related gene) proteinlerine bağlanarak repolarizasyonu geciktirdiği ve aksiyon potansiyeli süresini uzattığı gösterilmiştir. Birçok vaka sunumu ve retrospektif çalışmada kronik domperidon kullanımı ile QT intervalinde uzama saptanmıştır. Benzer şekilde çoğunlukla retrospektif verilerin meta-analizlerinde domperidon kullanımı ani kardiyak ölüm ve ventriküler aritmi riskinde artışa neden olabileceği gösterilmiştir. Bu yazıda kronik domperidon kullanımın kardiyovasküler sistem üzerine etkilerinin güncel derlemesi sunulmuştur.
