Abstract

Aim: It is known that accompanying peripheral arterial disease (PAD) increases mortality in patients undergoing open-heart surgery. Accordingly, it has been used for the calculation of risk by EuroSCORE II. However, as being a referral tertiary center, it is our clinical observation that EuroSCORE II significantly underestimates mortality much more than observed mortality in patients with PAD un- dergoing open-heart surgery. The purpose of the present study is to investigate the accuracy of our observation. Material and Method: One hundred and six patients undergoing open-heart surgery with varying diagnoses were divided into two groups as those with and without peripheral arterial disease. The predicted mortality was calculated by EuroSCORE II in the preoper- ative period, and then compared with the observed mortality. Results: The predicted and observed mortality rates were 1.3% and 21% in patients with peripheral arterial disease whereas these were 0.8% and 1.1% in patients without peripheral arterial disease, respectively. Conclusion: EuroSCORE II significantly underestimates mortality risk in patients with peripheral arterial disease undergoing open- heart surgery. Peripheral arterial disease may be related to more pronounced mortality increase much more than previously known.
