Tricuspid regurgitation is common in patients with mitral valve disease. Tricuspid regurgitation in this condition is usually functional, but can be organic, as well. We herein present a patient with mitral valve prothesis due to rheumatic mitral valve disease, in whom a flail tricuspid valve and associated severe tricuspid regurgitation were detected. A careful interpretation of the tricuspid valve should be made during transthoracic echocardiography, because the approach to functional and organic tricuspid regurgitation may differ from each other.
