Abstract

Objective: We investigated prognostic value of red blood cell distribution width to hematocrit ratio in ST-elevation myocardial infarction patients who were treated with primary percutaneous coronary intervention. Material and Method: A total of 650 ST-elevation myocardial infarction patients underwent primary percutaneous coronary interven-tion were included in this study. The patients were divided into two groups according to baseline red blood cell distribution width to hematocrit ratio: the high red blood cell distribution width to hematocrit ratio group (red blood cell distribution width to hematocrit ratio >0.3948) and low red blood cell distribution width to hematocrit ratio group (red blood cell distribution width to hematocrit ratio ≤ 0.3948). Primary endpoint was all-cause mortality. Results: Multivariate analysis showed that high red blood cell distribution width to hematocrit ratio was an independent predictor of all-cause mortality (HR: 2.175, 95%CI:1.393-3.397, p=0.001). The discriminatory performance of red blood cell distribution width to hematocrit ratio was better than both red blood cell distribution width and hematocrit for predicting all-cause mortality (Red blood cell distribution width to hematocrit ratio vs red blood cell distribution width: z=2.55, p=0.011; red blood cell distribution width to hemat-ocrit ratio vs hematocrit: z=2.83, p=0.005). Conclusions: Red blood cell distribution width to hematocrit ratio may be a new, useful, novel predictor of mortality in ST-elevation my-ocardial infarction patients who underwent primary percutaneous coronary intervention.

Özet

Amaç: Primer perkütan koroner girişim uygulanan ST elevasyonlu miyokard enfarktüsü hastalarında eritrosit dağılım genişliğinin hema-tokrite oranının prognostik değerini araştırdık. Gereç ve Yöntem: Çalışmaya primer perkütan koroner girişim uygulanan 650 ST elevasyonlu miyokard enfarktüsü hastası alındı. Hastalar bazal eritrosit dağılım genişliğinin hematokrite oranına göre iki gruba ayırıldılar; yüksek eritrosit dağılım genişliğinin hematokrite oranı grubu (eritrosit dağılım genişliğinin hematokrite oranı > 0,3948) ve düşük eritrosit dağılım genişliğinin hematokrite oranı grubu (eritro-sit dağılım genişliğinin hematokrite oranı ≤ 0,3948). Primer sonlanım noktası tüm nedenlere bağlı mortalite olarak değerlendirildi. Bulgular: Çok değişkenli analiz; yüksek eritrosit dağılım genişliğinin hematokrite oranının, tüm nedenlere bağlı mortalitenin bağımsız bir ön gördürücüsü olduğunu gösterdi (HR: 2,175, 95%CI:1,393-3,397, p=0,001). Eritrosit dağılım genişliğinin hematokrite oranının ayırım-cı performansı, tüm nedenlere bağlı mortaliteyi öngörmek için hem eritrosit dağılım genişliği hem de hematokritden daha iyiydi (eritro-sit dağılım genişliğinin hematokrite oranı - eritrosit dağılım genişliği: z = 2,55, p=0,011; eritrosit dağılım genişliğinin hematokrite oranı - hematokrit: z=2,83, p=0,005). Sonuç: eritrosit dağılım genişliğinin hematokrite oranı, primer perkütan koroner girişim uygulanan ST elevasyonlu miyokard enfarktüsü hastalarında mortalitenin faydalı, yeni bir belirtecidir.
