Abstract

Objective: Fractional flow reserve is one of the standard methods for assessing the severity of stenosis in the coronary artery. However, the factors contributing to the formation of maximal hyperemia in patients undergoing fractional flow reserve are not clearly defined. In this study, we investigated the effect of eosinophil levels on maximal hyperemia where the stenosis severity in coronary artery was assessed with fractional flow reserve. Material and Method: Our study was performed on patients with stable coronary artery disease who had intermediate coronary lesion after coronary angiography and who underwent fractional flow reserve measurement to evaluate the lesion severity. The patients were divided into two groups according to ischemia status after fractional flow reserve measurement and the role of eosinophil levels in the development of functional ischemia was investigated. Results: A total of 639 patients were included in the study, the mean age of patients was 62±8.1 years and 75% were males. In univariate analysis, while Syntax Score, C-reactive protein, eosinophil level, hypertension, neutrophil lymphocyte ratio, aspartat aminotransferas and high density lyphoprotein cholesterol were associated with the development of functional ischemia; in regression analysis, Syntax Score, eosinophil levels and neutrophil/ lymphocyte ratio were found to be independent predictors of functional ischemia. While there was a moderate correlation between eosinophil levels and basal distal coronary and aortic pressure value ratio, a good correlation was found between distal coronary and aortic pressure value ratio during maximal hyperemia. Conclusion: Although fractional flow reserve is a good diagnostic tool in the investigation of coronary ischemia, elevated histamine levels may cause false positive results in patients with high eosonophil levels. The use of additional imaging methods in such patients may be a more reasonable approach.
