Abstract

Purpose: Endophthalmitis is still the most frustrating complication of the ophthalmic surgery. This study aimed to evaluate clinical features, risk factors, treatment strategies and prognosis in patients with postoperative endophthalmitis. Material and Methods: Thirty nine eyes with postoperative endophthalmitis between January 1998 and April 2003 were prospectively evaluated. The conjunctiva, humor aqueous and vitreous samples were cultured in most of the cases. Patients were treated immediately with intravitreal antibiotic injection±pars plana vitrectomy. In culture positive cases without any clinical improvement within 36-48 hours, intravitreal antibiotic injection was repeated. Results: Endophthalmitis developed after cataract extraction and/or intraocular lens implantation in 33 eyes (84.6%). Glaucoma surgery (4 eyes, 10.2%), keratoplasty (1 eye, 2.6%), pars plana vitrectomy and cataract extraction (1 eye, 2.6%) were other causes of endophthalmitis. Culture from vitreous specimen was positive in only 48% of the patients. Intravitreal antibiotic had to be repeated in 4 patients. The presence of cystic bleb, posterior capsular rupture and diabetes were the risk factors for developing endophthalmitis. The visual acuity less than hand movements at first examination, delayed treatment, male sex and the presence of diabetes were the worse prognostic factors. Culture positivity had no effect on visual prognosis. Conclusions: To recognize the risk factors and take the measures are essentials in reducing the incidence of postoperative endophthalmitis. An early treatment with intravitreal antibiotic injection± pars plana vitrectomy is the best approach to improve the prognosis and maintain an acceptable final visual acuity.
