Abstract

PURPOSE: It has been shown that a part of humor aqueous production directs posteriorly and this fluid is transported by retina pigment epithelium to choroidal circulation, especiaily in pathologic conditions such as a break in retina. Since the macular hole may represent a similar model, we investigated the effect of this transvitreal flow on intraocular pressure (IOP) in idiopathic macular hole cases. MATERIAL AND METHODS: In grade III and IV unilateral macular hole cases referred to our retina department, retinal examination and intraocular pressure measurement was done by different ophthalmologists. The patients who have been expected to have IOP abnormalities were excluded. The results were compared with the pressures of age-matched patients who have no disease other than simple refractive disorders. RESULTS: The average IOP was 13.86 ± 3.09 mmHg in the affected eyes, and 15.82 ± 2.34 mmHg in the fellow eyes of study group, consisted of 22 macular hole cases. The difference was 3.77 ± 2.22 mmHg (paired t test, p=0.000, significant). The average IOP was 14.44 ± 1.72 mmHg in right eyes and was 14.53±1.61 mmHg in left eyes of control group of 32 cases. The difference between the both eyes was 0.41±0.56 mmHg (paired t test, p=0.22, not significant). The overall pressure differences between the both eyes of macular hole group (3.77 ± 2.22 mmHg) and control group (0.41 ± 0.56 mmHg) was statistically significant (unpaired t test, p=0.000). CUNCLUSION: Considering the results of this study, it can be concluded that the IOP in eyes with macular hole is 3.77 ± 2.22 mmHg lower than their fellow eyes, the relationship of this finding with production rate of humor aqueous and transvitreal fluid transport should be further evaluated.
