Abstract

Aim: To evaluate outcomes of phacoemulsification surgery with 2.2 mm clear corneal incision and compare the results with that of 2.8 mm phacoemulsification cases. Material and Methods: Seventy-four eyes of 57 patients who hadphacoemulsifkation with 2.2 mm clear corneal incision (group I) and 28 eves of 25 patients who had 2.8 mm incision surgery (group II) in our Cataract and Refractive Surgery department were included in the study. Mean postoperative follow-up period was 152.1±55.1 and 146.1±46.5 days, respectively. Peroperalive mean phaco time (PT). phaco time percentage (AP%) (MPTP). effective phaco lime (EFT), total operation time (TOT) and peroperative complications were recorded. Besides routine ophthalmic examinations on postoperative 1st day. 1st week, 1st month and 3rd month, pachymetry and contrast sensitivity measurements were also performed. Surgically induced astigmatism on postoperlive 7 th day, 1st month and 3rd month were calculated with Naeser 's polar value method and vector analysis. Results: Mean age was 67.0±11.1 years in Group 1 and 67.3±14.2 years in Group II (p>0.05). Parameters recorded during operation were TOT 12.8±3.1 min. FT 0.26±0.2 min. MPTP (AP%) % 13.64 ±7.6 and EPT 2.86±5.4 sec in Group I and 14.03±5.0 min. 0.31±0.2 min. %13.87± 2.96±2.8 sec in Group II respectively. Statistically significant difference was not observed (P>0.05). Posterior capsular rupture in 2 cases of Group I and I case of Group II. zonular dialysis in a patient in Group I and iris damage in a patient in Group II was occurred. There were no difference according to visual acuities and contrast sensitivity values in 3, 6, 12 and 18 cpd between two groups(p>0.05). When evaluated for surgically induced astigmatism calculated by Xaeser's polar value method and vector analysis, there was no difference between groups except postoperative 7th day (Naeser: -0.35±0.S7 in Group I, -l.2l±l.84 in Group II). There was also no statistically significant difference in preoperative and postoperative 3rd month astigmatism values in Group I (p>0.05). Conclusion: When operations with 2.2 mm incision arc compared with 2.8 mm incision ones, they do not create additional risks both for peroperative and postoperative period and thev also provide better but statistically insignificant results for surgically induced astigmatism.

Özet

Amaç: 2.2 mm saydam kornea kesişi yapılan fakoemülsifikasyon cerrahisi sonuçlarının değerlendirilmesi ve 2,8 mm keşiden yapılan olgularla karşılaştırılması. Gereç ve Yöntem: Kliniğimiz Katarakt ve Refraktif Cerrahi biriminde 2.2 mm saydam karneni keşi ile fakoemülsijikasyon yapılan 57 hastanının 74 gözü ile (grup I), 2.8 mm'den fakoemülsifikasyon gerçekleştirilen 25 hastanın 28 gözü (grup II) çalışmaya alındı. Ortalama takip süreleri gruplarda sırasıyla 152.1± 55.1 ve 146.1±46.5 gün idi. Ameliyat öncesi ortalama faka zamanı (FZ), fako gücü yüzdesi (%AP) (FGY),etkili fako zamanı (EFZ), tüm ameliyat zamanı (TAZ) ve ameliyat içi komplikasyonlar kaydedildi. Tüm olgularda ameliyat öncesi ve sonrası 1. gün, 1. hafta, 1. ay ve 3. ayda rutin göz muayenesinin yanında; pakimetri ve kontrast duyarlılık ölçümleri yapıldı. Ameliyat sonrası 7 gün, 1. ay ve 3. aylarda meydana gelen cerrahi uyarılmış astigmatizma değerleri Naeser' in polar değerlendirme ve vektör analiz yöntemleriyle hesaplandı. Bulgular: Ortalama yaş I. Grup'ta 67.0±11.1, II.
