Abstract

Aim: To evaluate the results of revision endoscopic dacryocystorhinostomy (END-DCR) surgery performed with topical mitomycin C (MMC) in patients with acquired nasolacrimal duct obstruction who had failed external dacryocystorhinostomy (EX-DCR). Material and Metod: Twelve eyes of 12 patients with failed EX-DCR were involved in this prospective study. During the END-DCR the obstructed mucosal tissue was excised and for some patients the bony window was enlarged. If needed silicone tube was inserted. Topical 0.02% MMC was applied for 3 minutes. The hemorrhage which occured during surgery was graded as mild, moderate, and severe. The lacrimal lavage, complications, and the symptoms were evaluated on the first postoperative day. Additionally the endoscopic evaluation of the nasal mucosal opening was performed on the first and third months. Results: There were 8 (66%) female, and 4 (34%) male with and average age and followup period of 63.7 (39-79) years and 4.2 (3-9) months respectively. One patient had septoplasy, 2 had silicone tube intubation. During surgery 10 (83%) patients had mild, and 2 (17%) had moderate hemorrhage. The mean mucosal opening was measured as 8.3 mm (6-10 mm) during surgery. After 1 month it was 6.4 mm (between 5-8 mm). After 3 months it was reduced to 6.1 mm (between 4-8 mm). The nasolacrimal system was patent in 11 (%91.6) patients at first postoperative week, and at 1st and 3rd postoperative months, while 1 (%8.4) patient had obstructed nasolacrimal system. During the follow-up period none of the patients had any serious complications. Conclusion: We demonstrated a success rate of 91.6% in revision END-DCR performed with topical MMC. Further studies with large number of patients and long follow-up periods are needed.

Bulgular: Olguların 8'i (%66) kadın, 4'ü (%34) erkekti. Yaş ortalamaları 63,7 (39-79) yıl, ortalama takip süreleri 4,2 (3-9) ay idi. Bir olguya septoplasti, 2 olguya silikon tüp entübasyonu yapıldı. Cerrahi sırasında 10 (%83) olguda hafif, 2 (%17) olguda orta düzeyde hemoraji görüldü. Ortalama mukozal açıklık miktarı END- DSR sırasında 8,3 mm (6-10 mm arası), 1. ayda 6,4 mm (5-8 mm arası), 3. ayda 6,1 mm (4-8 mm arası) olarak ölçüldü. Cerrahi sonrası 1. hafta, 1. ve 3. aylarda yapılan punktum lavajı 11 (%91,6) olguda açık iken 1 (%8,4) olguda kapalı idi. Olgularda takip süresince ciddi bir komplikasyon gözlenmedi Sonuç: Çalışmamızda topikal MMC kullanılarak yapılan revizyon END-DSR cerrahisi sonrası %91,6 oranında başarı elde edilmiştir. Geniş olgu serileriyle, uzun takip süreli çalışmalar yapılmalıdır.
