Abstract

Aim: To compare extracapsular cataract extraction (ECCE) and phacoemulsification (PHACO) surgery techniques in visual, refractive and effects on the corneal endothelium in elderly patients. Material and Method: Thirty eyes of thirty patients older than 80 years old underwent cataract surgery due to senile cataract were included in the prospective study. Preoperatively complete ophthalmic examination as well as grading of hardness of the cataract and endothelial cell count (ECC) with specular microscopy were done. Randomly, fifteen eyes had ECCE (Group 1) and fifteen eyes had phacoemulsification (Group 2) procedures. Postoperatively complete ophthalmic examination and endothelial cell count were performed at the 1st, 2nd, 6th, 18th month and statistical analysis was performed. Results: All patients were followed-up 18 months. In group 1, the mean best corrected visual acuity (BCVA), sphercal equivalent (SE) and endothelial cell count were 0.14±0.06 (mean±standard daviation), 0.83±0.41 diopter (D) and 2289±681 cell/mm² respectively. In Group 2 same values were 0.17±0.07, 0.85±0.39 D and 2428±672 cell/mm² respectively. Postoperatively 18th month, BCVA, SE and ECC were 0.77±0.09, 0.90±0.38 D and 1904±647 cell/mm² respectively in Group 1. In Group 2 BCVA, SE and ECC were 0.70±0.19, 0.87±0.37 D and 1823±618 cell/mm² respectively. When postoperative and preoperative values were compared between two groups, decrement in ECC was found to be significantly higher in group 2 (p<0.05). Conclusion: Comparison of ECCE and PHACO procedures in the effect of the corneal endothelial cell count, ECCE was found to be safe procedure in the cataract patients older than 80 years old.
