Abstract

PURPOSE: To determine the neurological involvement rate, clinical manifestations and prognosis in the patients with Behçet's disease. MATERIALS AND METHOD: We have reviewed the charts of the patients with ocular involvement associated with Behçet's disease who were followed in our uveitis clinic between 1990-1997. Diagnosed as Neuro-Behçet, 1 female and 9 male patients were included in the study group. The age at the initial symptoms of Behçet's disease, the age at the beginning of the neurological involvement, clinical manifestations, results of the HLA testing, visual evoked potentials (VEP), computerized tomography (CT) and magnetic resonance imaging (MRI) examination were evaluated. Best corrected visual acuities (BCVA) and the ophthalmological findings were recorded. The patients who have no examination in the last 6 months were called to have a new one. RESULTS: Neurological involvement rate was found as %2.79 in Behçet's disease with uveitis. Male/female ratio was 9/1. The initial Behçet's disease symptoms started at the mean age 24±1.8. The neurological involvement was seen 4.7±0.79 years after the disease had begun. The neurologic findings: pyramidocerebellar Involvement in 3, pyramidal involvement in 3, brain stem involvement in 1, cerebellar involvement in 1, paraparesis in 1 and twelfth cranial nerve involvement in 1 case. In 8 patients initial CT and MRI studies were defined as normal. After the repetition of these examinations the lesions were found in 7 patients. One patient had an improvement, 4 had progression and 5 had no change in neurological findings. BCVA increased in 5, decreased in 6 and didn't change in 8 eyes. CONCLUSION: Neurological involvement is more frequent in male. Neurological consultation have to be performed in every patient with Behçet's disease since this complication effects the prognosis. CT and/or MRI examinations should be repeated , if the initial results are in normal limits in patients who have neurological symptoms and/or findings.
