Abstract

Aim: To assess the efficacy and safety of tacrolimus in prevention of allograft rejection in high risk limbal grafts. Material and Methods: Six eyes of six patients with severe limbal stem cell deficiency were included in this prospective study. Limbal allograft transplantations were performed between July 2002 and April 2005. All of the patients were given 0.1mg/kg/day tacrolimus orally 3 days before the surgery. Primary diagnosis was alkali burn in five patients and keratoconjunktivitis sicca secondary to Stevens- Johnson syndrome in one patient. All patients were monitored for efficacy and drug side effects postoperatively. Results: The mean follow up was 49.2 months (range, 38 to 65 months). Postoperatively, outgrowths of corneal epithel from limbal grafts started within a week. The mean epithelial healing time was 14.2±5.8 days (range, 9 to 22 days). In three cases, penetrating keratoplasty was performed mean 5 months (range, 3 to 10 months) after limbal transplantation. In three cases, the follow up was complicated with several episodes of epithelial defects. Therefore amniotic membran transplantation was performed in these cases. During the follow up period, 5 eyes showed improvement in vision, and one eye had deterioration in vision. Side effects related to tacrolimus were not observed. Tacrolimus was discontinued at a mean of 6.16 months (range 3 to 8 months). Conclusions: Allo-limbal transplantation for ocular surface reconstruction has a successful outcome with using systemic tacrolimus for immunosupression. It is a valuable option for patients with severe bilateral limbal stem cell deficiency.
