Abstract

Purpose: To determine the effectiveness and safety of a vital stain, trypan blue, to provide a regular capsulorhexis for phacoemulsification surgery in cataracts with no visible fundus reflex. Material and Method: Under local anesthesia and temporal clear corneal insicion, capsulorhexis was performed after staining the anterior capsule of the lens with 0.1% trypan blue in 88 cases. After capsulorhexis and hydrodissection, nucleus was emulsified and intra ocular lens was implanted. Controls were done 1 day, 1 week, and 1 month after the operation with visual acuity measurment, biomicroscopic examination and intraocular pressure measurement with air puff tonometer. Results: In all cases capsulorhexis edge was easily visualised after staining the capsule with trypan blue. Radial tears occured in 4 cases during the capsulorhexsis. In all other cases capsulorhexis was completed without any complication. Anterior vitrectomy was done in 6 cases because of posterior capsule tear. In 2 cases anterior chamber IOL, and in 4 cases sulcus IOL implantation required. In all other cases IOL was implanted in the bag. No complication related to trypan blue was observed during the operation and in the follow up period. During the early postoperative period we have observed stromal edema in 11 cases, iris atrophy related to trauma in operation in 4 cases, and corneal incision site burn in 1 case. Also we detected pigment dispersion in 1 case, exudative reaction in anterior chamber in 2 cases, and transient IOP elevation in 6 cases. Conclusion: A smooth edged and regular capsulorhexis can be performed with the aid of trypan blue, so that a safe phacoemulsification surgery can be done in mature cataracts in which retinal reflex cannot be seen.
