Abstract

Aim: To evaluate efficacy of intravitreal pegaptanib sodium injection in choroidal neovascularization (CNV) secondary to age-related macular degeneration (AMD). Material and Method: Data of 23 patients with CNV secondary to AMD were retrospectively evaluated. Best-corrected visual acuity (logMAR), intraocular pressure, biomicroscopy, and optical coherence tomography (OCT) were evaluated before and every 6 weeks after injection. Fundus fluorescein and indocyanine green angiography were performed every 3 months. Results: Twenthy-six eyes of 23 patients with AMD were included. Mean age was 74.5 years (range; 64 to 87 years). There were occult CNV in 16 eyes, predominantly classic CNV in 6 eyes and minimally classic CNV in 4 eyes. Of the eyes with occult CNV, 9 had focal spot, 4 had idiopathic polipoidal choroidal vasculopathy, and 3 had retinal angiomatous proliferation. Photodynamic theraphy (PDT) was performed before pegaptanib sodium injection in fifteen eyes. Lesion size was ≤4 disc area in 24 eyes. The mean best-corrected visual acuity (BCVA) was 0.76 ± 0.37 logMAR at the first visit and 0.80 ± 0.41 logMAR at the last visit. The first mean BCVA was not significantly different from the mean BCVA at third, sixth, twelfth and eighteenth months respectively (p>0.05). Re-injection was performed when there was persistance or increase in intraretinal/subretinal fluid and loss of at least one line of BCVA. Injections were performed every six weeks. The mean number of injection was 2.48 (range;1 to 6) and mean follow-up was 10.24 months (range;3-30 months). Submacular hemorrhage occurred 3 weeks after the first injection in one eye and 5 weeks after from the second injection in two eyes. Endolphthalmitis, uveitis, retinal detachment, traumatic lens injury or tromboembolic events were not observed. Conclusion: Injection of intravitreal pegaptanib sodium in eyes with CNV secondary to AMD caused stabilization of vision and decrease in membrane activation during one-year follow-up.
