Abstract

Purpose: To investigate tear changes and other complications in early period of systemic isotretinoin therapy. Material and Methods: Twenty subjects beginning systemic isotretinoin for nodulocystic acne were included in the study. Patients were evaluated in terms of Schirmer 1 test, tear film break up time, presence of subjective complaints, and corneal staining with fluorescein before and one month after isotretinoin treatment, prospectively. Color visions of subjects were tested and their night visions were asked. Schirmer score under 10 mm and tear film break up time under 10 seconds were accepted as abnormal. Statistical analyses were performed with Wilcoxon test and chi-square test. Results: At the first month of the treatment, Schirmer scores decreased from 19.2±7.3 to 13.2±6.6 mm (p<0.05) and tear film break up time decreased from 16.9±2.9 to 11.1±3.4 seconds (p<0.05). At the end of first month, Schirmer test in 13 (65%) cases and tear film break up time in 10 (50%) cases yielded abnormal results. Subjective complaints developed in eight (40%) cases related to dry eyes. Any change in visual acuity, color vision, or night vision of the subjects didn't develop. Conclusion: Dry eye symptoms and signs appear in the first month of the isotretinoin treatment. In cases considering isotretinoin treatment, we think that supplementary artificial tears may be helpful.
