Abstract

Acute exacerbations of chronic obstructive pulmonary disease (COPD) are a common cause of admission to hospital and have a high mortality. Nazal positive pressure ventilation (NPPV) has been used succesfully in patients with respiratory failure due to COPD as well as neuromuscular and skeletal disorders. We evaluated the efficiency of NPPV in patients with hypercapnic repiratory failure due to COPD. NPPV was given to 23 patients with acute hypercapnic respiratory failure in addition to the standard medical therapy. Arterial blood gas analyses, measurements of respiratory rate, blood pressure and heart rate were made immediately after admission to the intensive care unit, one hour after admission, six hours after admission, at 24 hours and on the day of weaning. On admission, the patients had a mean partial pressure of oxygen in the arterial blood (PaO2 of 40.01±10.04 mmHg, the partial pressure of carbon dioxide in the arterial (PaCO2 of 78.99±l2.37 mmHg and forced expiratory volume in 1 second (FEV1) revealed %39±%11 and respiratory rate of 31±10 breaths/min. The mean duration of hospital stay was 15 days. After the beginning of NPPV treatment, PaCO2 was found to be significantly (p< 0.01) decreased even on the first hour of admission. PH and PaO2 was found to be significantly (p<0.01, p<0.05) increased after one hour of ventilator treatment. A significant (p<0.05) decrease in respiratory rate occured on the sixth hour of admission. This study suggests that early application of NPPV in acute hypercapnic respiratory failure patients facilitates improvement and decreases the need for invasive mechanical ventilation.

Özet

Hastaneye en sık yatış ve mortalite sebeplerinden, biri kronik obstrüktif akciğer hastalığı (KOAH) akut alevlenmedir. Nazal pozitif basınçlı ventilasyon (NPPV) nöromuskuler ve iskelet bozukluklarına ek olarak KOAH'a bağlı solunum yetmezlikti hastalarda başarıyla kullanılmaktadır. Bu çalışmada KOAH' a bağlı hiperkapnik solunum yetmezlikti hastalarda NPPV'ın etkinliğini değerlendirdik. Akut hiperkapnik solunum yetmezlikli 23 hastaya standard tıbbi tedaviye ek olarak NPPV verildi. Hastaların yoğun bakım ünitesine (YBÜ) alındıkları anda, NPPV'nın L, 6., 24. saatlerinde ve NPPV den ayrıldığı günde atreriyel kan gazı analizleri, solunum hızı, kan basıncı ve kalp hızı ölçümleri kaydedildi. Hastaların YBÜ'ne kabullerinde ortalama arteriyel kanda parsiyel oksijen basıncı (PaO2): 40.01±10.04 mmHg, arteriyel kanda parsiyel karbondioksit basıncı (PaCO2): 78.99±12.37 mmHg,birinci saniyedeki zorlu ekpiratuar volüm (FEV1): Beklenilen değerin %39 ± %11'i, solunum hızı: 31±10 solunum/dk idi.
