Abstract

Aim: To evaluate the success of penetrating keratoplasty in the treatment of corneal perforations due to corneal diseases. Material and Method: We retrospectively analyzed 51 eyes of 51 patients who underwent penetrating keratoplasty to restore anatomic integrity of the eye. The mean follow-up time after keratoplasty was 41.2±8.4 (6-108) months. Predisposing conditions leading to perforation were microbial keratitis in 27 eyes (52.9%), corneal melting associated with ocular surface disorder in 21 eyes (41.2%) and perforated descemetocele in 3 eyes (5.9%). The results were evaluated for the following criteria; anatomical stability of the eye, graft clarity, complications and visual acuity. Results: Anatomical stability was achieved in 49 (96.1%) of the eyes. The graft clarity rate was 64.7%. The most common complication was glaucoma in 15 of the eyes (29.4%). Twenty five eyes (49%) obtained a final refracted visual acuity of 20/100 and beter. Conclusion: Emergency penetrating keratoplasty may be necessary to restore anatomic integrity in large corneal perforations. The prognosis for graft clarity is usually poor in these cases. On the other hand without penetrating keratoplasty, these eyes would have been lost. Prognosis can be improved by the control of the underlying pathology, inflammation and intraocular pressure.
