Abstract

Aim: To investigate the etiology, gender, age, accompanying eye examination findings and result of surgery in cases with spontaneous vitreous hemorrhage. Material and Method: Four hundred forty-five eyes of 445 patients with spontaneous vitreous hemorrhage that were operated in Turkish Ministry of Health Ankara Ulucanlar Eye Education and Research Hospital Retina Clinic between June 2004 and May 2008 were retrospectively evaluated in this study. A complete ophthalmological examination including visual acuity, intraocular pressure with applanation tonometry, slit-lamb biomicroscopy, fundoscopy and ocular ultrasonography were performed at preoperative and postoperatively. Results: There were 234 female (52.6 %) and 211 male (47.4 %). The average age of patients was 58.5 years (17-88 years). The etiology was proliferative diabetic retinopathy in 369 eyes (82.9 %), retinal vein occlusion in 62 eyes (13.9 %), and age-related macular degeneration in 14 eyes (3.2 %). All patients were operated with standard vitrectomy technique with three-port tunnel sclerotomies. Conclusion: Based on the results that were obtained in this series, proliferative diabetic retinopathy was the most common cause in cases with spontaneous vitreous hemorrhage. Improvement of visual acuity after vitreoretinal surgery was mostly seen in spontaneous vitreous hemorrhage with retinal vein occlusion.

Özet

Amaç: Spontan vitre içi kanama tanısı ile ameliyat edilen olguların etiyoloji, yaş, cinsiyetözellikleri, eşlik eden göz muayene bulguları ve uygulanan cerrahinin sonuçlarını incelemek.Gereç ve Yöntem: Haziran 2004 ve Mayıs 2008 tarihleri arasında Sağlık Bakanlığı An ka ra Ulucanlar Göz Eğitim ve Araştırma Hastanesi Retina bölümünde spontan vitre içi kanamatanısı alarak ameliyat edilen 445 hastanın 445 gözü geriye dönük olarak incelendi. Ameliyatöncesi ve sonrası görme değerleri, göz içi basıncı değerleri, biyomikroskopi bulguları, pupildilatasyonu sonrası göz dibi muayenesi ve oküler ultrasonografiyi içeren tüm göz muayenebulguları değerlendirildiBulgular: Çalışmaya alınan olguların 234'si kadın (% 52,6), 211'i erkek (% 47,4) idi.Olguların ortalama yaşı 58,5 yıldı (17-88 yıl). Etiyolojik neden olarak 369 gözde (% 82,9)proliferatif diyabetik retinopati, 62 gözde (% 13,9) retina ven tıkanıklığı ve 14 gözde (% 3,2)yaşa bağlı maküla dejeneresansı mevcuttu. Tüm hastalar üç girişli tünel sklerotomisi uygulanan standart vitrektomi tekniği ile ameliyat edildi.Sonuç: Bu serideki sonuçlara göre, spontan vitre içi kanama olgularında en sık nedenproliferatif diyabetik retinopatidir. Vitreoretinal cerrahi sonrası görme keskinliğinde artış enfazla retina ven tıkanıklığına bağlı vitre içi kanama olgularında gözlemlenmiştir
