Abstract

Objective: In this study, we aimed to evaluate the results of aponeurotic and frontalis sling surgery performed for congenital ptosis in terms of surgical success. Material and Method: Sixty nine eyes of 52 patients, aged 15-72 years, who underwent surgical treatment for congenital ptosis between October 2010 - February 2013 in Haydarpaşa Numune Training and Research Hospital Ophthalmology Clinic Oculoplastic Surgery Unit were retrospectively reviewed. Results: Fifty two patients, 35 male (67.4%) and 17 female (32.6%) (average age; 21± 0.6 years), with congenital ptosis were included in the study. Seventeen patients had bilateral (32.6%) and 35 patients had unilateral ptosis (67.4%). Thirty eight eyes of 26 patients had congenital myogenic ptosis (50%), 28 eyes of 23 patients had congenital aponeurotic ptosis (44.3%), and 5 eyes of 3 patients had congenital neurogenic ptosis (5.7%). Frontalis suspension surgery was performed in patients with a levator function less than 5 mm, a prominent behind eyelid (lid-lag) or an absent skin crease, and levator aponeurotic surgery was performed in patients with a levator function over 5 mm. One eye of 3 patients with congenital neurogenic ptosis underwent aponeurotic surgery, and 2 patients with Marcus Gunn Syndrome underwent levator extirpation combined with frontalis suspension surgery. Recurrence rates after frontalis suspension and aponeurotic surgery were 7.2% and 16.7%, respectively. Strabismus surgery for left esotropia was simultaneously performed for one patient who underwent aponeurotic surgery. Conclusion: Levator aponeurotic surgery was performed in patients with good levator function, frontalis supension surgery was performed in patients who had poor levator function and prominent ptosis. A significant difference in recurrence rates was found between ptosis surgery procedures based on the levator function.

Özet

Amaç: Doğumsal ptozis nedeniyle uygulanan levator aponevroz ve frontal askılama cerrahisi sonuçlarını cerrahi başarı açısından değerlendirmeyi amaçladık.Gereç ve Yöntem: Haydarpaşa Numune Eğitim ve Araştırma Hastanesi Göz Hastalıkları Kliniği Oküloplasti Birimi'nde Ekim 2010-Şubat2013 tarihleri arasında doğumsal ptozis nedeniyle cerrahi tedavi uygulanan 15 ay-72 yaş arası 52 hastanın 69 gözü retrospektif olarak inceledi. Bulgular: Yaş ortalaması 21±0,6 yıl olan 35' i erkek (%67,4), 17'si kadın (%32,6) 52 doğumsal ptozis olgusu çalışmaya alındı.
