Abstract

Objectives: To investigate the effect of age on bevacizumab response in treatment-naive diabetic macular edema. Material and Method: Ninety-eight eyes of 98 treatment-naive patients with macular edema secondary to diabetes mellitus were enrolled in this study. The patients in the study were divided into the following three groups according to their ages: group 1 (40-50 years), group 2 (51-60 years), and group 3 (>60 years). Three consecutive injections at intervals of 1 month were applied to all diabetic patients. The efficacy of the bevacizumab treatment on macular edema according to age groups were assessed by optical coherence tomography by comparing the central foveal thickness and by mean visual acuity changes after initial and three loading dose injections. Results: After three consecutive bevacizumab injection, the mean reduction of central foveal thickness in groups 1, 2, and 3 were - 244.1±103.4, -202.7±98.9 and -194.7±101.6 μm, and mean improvement of visual acuity in groups 1, 2, and 3 were 6.9±0.8, 5.4±0.5, and 5.2±0.6 ETDRS letters respectively. The improvement of visual acuity and reduction of central foveal thickness was significantly different in each group (p<0.001 in all groups, Paired-samples t-test). The changes of central foveal thickness and visual acuity was significantly different in group 1 from other groups (p=0.024 and p=0.032 respectively, Analysis of covariance- ANCOVA). Additionally, ages of patients were correlated with the mean reduction of central foveal thickness and mean improvement of visual acuity for the whole study group (r=-0.290, p=0.036 for central foveal thickness, r=-0.610, p=0.019 for visual acuity, Pearson correlation). Conclusion: In this study, it was observed that the efficacy of bevacizumab treatment was found to be more effective in younger patients in treatment-naive diabetic macular edema patients.
