Abstract

Macular hole is the local loss of foveal retinal layers causing severe central vision loss. Traumatic macular hole is the second most com- mon type of macular hole after idiopathic macular hole. In this study; we aimed to present a case of choroidal rupture and traumatic macular hole which developed after trauma. 24-years-old-male referred to our clinic with blunt trauma in the left eye. His best corrected visual acuity was counting fingers at two meters in the left eye. Ophthalmological examination revealed extensive intraocular hemorrhages and choroidal rupture, and on the follow-up period full-thickness traumatic macular hole and cataract was diagnosed on the fifth day and the width of the traumatic macular hole was 678 microns. He underwent pars plana vitrectomy with internal limiting membrane peeling and gas tamponade combined wtih cataract surgery. After one month visual acuity was improved to 3/10. Although a standardized approach to traumatic macular hole is not available yet, we think that if the visual acuity is low, the patient is older than 23 years age, hole is wide, tangential traction con- tinues in the hole area and if the hole does not regress during follow up, early vitreoretinal surgery is helpful and combined surgery is safe.
