Abstract

Aim: The aim of this study was to investigate whether left ventricular hypertrophy is associated with P wave duration and P dispersion (PD) in subjects with and without coronary artery disease. Methods and Results: One hundred nineteen individuals who underwent coronary angiography and without history of myocardial infarction were enrolled into the study. In all individuals, 12-lead surface electrocardiograms were recorded, and maximum and minimum P wave duration (Pmax and Pmin) and PD were calculated (PD=Pmax-Pmin). Left ventricular mass index (LVMI) was calculated by echocardiography in all subjects. Left ventricular hypertrophy (LVH) was defined as LVMI>134 gr/m2 in men and >110 gr/m2 in women. Result: In 55 (Group 1-A) of 85 individuals who had normal coronary angiogram (Group 1), LVH was absent, and it was present in 30 (Group 1-B) of them. In 15 (Group 2-A) of 34 patients who had $\geq$50% stenosis at least in one major epicardial coronary artery (Group 2), LVH was absent, and it was present in 19 (Group 2-B) of them. Pmax and PD were higher, and Pmin was lower in Group 2 compared with Group 1 (115±11 ms and 109±11 ms, P=0.011; 35±7 ms and 26±7 ms, P<0.001; 80±7 ms and 84±9 ms, P=0.049; respectively). In the comparison of the subgroups, while PD was significantly higher in Group 1-B compared with Group 1-A (28±8 ms and 25±6 ms, P=0.037; respectively), there was no significant difference between Group 2-A and Group 2-B (P>0.05). In all subjects, LVMI was significantly correlated with PD (r=0.301 and P=0.001). However, when correlation test was performed one by one in Group 1 and Group 2, although LVMI was correlated with PD in Group 1, not in Group 2 (r=0.319 and P=0.003; r=0.116 and P=0.515; respectively). Conclusion: While PD is related to LVMI in individuals without coronary artery disease, it is not in patients with coronary artery disease.
