Abstract

: Reversing of neuromuscular blockade of mivacurium with anticholinesterases is not clear. We aimed to compare the reversal of the superficial neuromuscular block provided by mivacurium with two different doses of neostigmine and the recovery time to spontaneous recovery. After the approval of the Ethic Committee and written informed consent, 45 patients aged 18-60, ASA phsycal status I-II, undergoing minör lower extremity operations were included. No premedication was given. All patients were monitorized with ECG, NIBP and neuromuscular monitorization were provided by TOF(Guard). Anesthesia was induced by 5-7 mg/kg thiopental, 1-2 mcg/kg fentanyl and maintained by 1-1.5 % isoflurane,60% N2O+40 %O2. After the calibration of TOF(Guard) 0.2 mg/kg mivacurium was given in 30 seconds when TOF was 0 the entiibation was performed. After this, the patients were randomly divided into 3 groups. When T%25 recovery was obtained group SP (n=15) was left to spontenouus recovery where in group N125 (n=15) 0.02 mg/kg neostigmine+0.01 mg/kg atropine was given and in group N225(n=15) 0.06 mg/kg neostigmine + 0.03 mg/kg atropine was used as a reverse. In all groups recovery times of Tmax, T25, T75 and T95, TOF70, recovery index and reversal time were recorded. Heart rate and NIBP were monitorized every 5 minutes. Changes in heart rate were recorded before and after the reversal. For statistical analyzes, Kruskall Wallis one way varyance analysis, Mann Witney U test, Pearson X2 test and Wilcoxon paired two sample method were used. In group N125 and N225 statistically significant decreases in heart rates were seen after the reversal of the block. In group N225 recovery index were significantly decreased- TOF70 and the reversal time values in group N125 and N225 were significantly shorter. In group N225 the reversal time was significantly shorter compared to group N125. We concluded that the reversal of the superficial neuromuscular block provided by mivacurium with neostigmine shortens the recovery time. Our data show that 0.06 mg/kg neostigmine achieves a good recovery without reducing heart rate.i
