Abstract

Aim: To repair a urethrovaginal defect in childhood is a challenge for a pediatric surgeon and treatment options are limited. Martius flap is a labial fat pad flap with a pudental vascular supply. Interposition of this flap between urethra and vagina provides a supportive tissue with a good blood supply. Here we report two girls who had Martius repair for their ıırethrovaginal defects. Method: Patients used used topical estriol and asiaticoside perineally for 3 weeks to reinforce the tissues. Martius repair was done using one sided labial fat pad flap. Urethral and bladder catheters were inserted. Urethra! catheter was removed on postoperative 14th day and bladder catheter on 21st day after controlling residual urine. Oxybutinin HCl was begun on postoperative 14th day. Results: First patient, 6 year old girl had lipomeningocele repair at the age of 18 months and had an iatrogenic uretherovaginal fistula due to catheter insertion. She developed urinary incontinence and three primary repair attempts were unsuccessful. Second patient was a 5 year old girl who had pouch colon with persistant cloacal malformation and had posterior anorectovaginourethroplasty. The urethrovaginal septum remained open. One attempt of primary repair was unsuccessful. Both patients were incontinent. Martius repair was done to both patients. Physical examination and cystoscopy on postoperative 3rd month revealed complete healing of the urethrovagi-nal defect. Urodynamic studies showed functional recovery in both patients. Conclusion: Martins fat pad flap repair is successful in repairing urethrovaginal fistulas in girls, it does not cause a donor area deformity, and it is easily prepared. The dimensions of the graft are suitable to repair wide fistulas.
