Abstract

Aim: Rapid dispersal of local anesthetics out of the wound infiltration area cause short analgesia time. We investigated the absorption rate and tissue reaction of subcutaneously placed bupivacaine soaked absorbable gelatin sponge in an experimental model. Method: In 10 anesthetized rats, a skin incision was made on both sides of the right and left inguinal area. On the right side, bupivacaine and 99mTc-pertechnetate mixture solution soaked absorbable gelatin sponge was placed subcutaneously, and on the left side, same solution was infiltrated. Sequential anterior scintigrams of the body were obtained at 15, 30, 60 min after surgical procedure, and at 2, 6, 24 hr, and statistically analyzed. All incisions were histopathologically evaluated on 21st day after surgery. Results: Scintigraphic images showed that in the infiltration side, more rapid dispersal of mixture solution out of the subcutaneous area while most of the remaining radioactivity persisted in absorbable gelatin sponge side even at the end of 24 hours. In all times, radioisotope counts were significantly high in right side when compared with left side (p<0.05). The results showed that absorbable gelatin sponge was well tolerated by subcutaneous tissue. Conclusion: This study would suggest that absorbable gelatin sponge might also be useful as an artificial carrier for local anesthetics when placed subcutaneously.
